 To enroll provide: A completed MAC PRO Membership Application Select only one Category/ Profession. Your choice will determine what level of discount you will receive. Makeup Artists receive 40% and all other categories receive 30%. You will have to prove your selection by providing two accepted pieces of professional criteria that indicate your chosen profession. Categories are: Makeup Artist, Aesthetician, Cosmetologist, Performers/On-Air Talent, Fashion Stylist/Costume Designer, Photographer, Model, Hairstylist, Nail Technician. If you have been a member in the past or are renewing please provide your existing PRO ID in the Membership Status section. If you provide your email address and if accepted, it will allow us to more quickly send your PRO ID with instructions of how to start enjoying your new benefits. A copy of Photo Identification and two pieces of Professional Criteria Provide two current pieces from the following that indicate your selected category/profession: Composite Card, Business Card with name and specific profession, Editorial Page with name credit, Union Card, Professional License, Diploma/Certificate, Publication Masthead, Program/press Materials with name, Contract on Production Company Letterhead, Crew/Call List on Prod. Co. Letterhead, Professional Letter of Reference of Employment. A Head Shot & Resume can only be used by Model or Performer/On-Air-Talent applicants. We do not accept business licenses, websites, pay stubs or tax forms as Professional Criteria. Annual Membership Fee: ($35 US / $45 CDN) payable by check, money order or credit card You can drop everything off at any M·A·C store for forwarding or send it directly to the processing department at: M·A·C PRO MEMBERSHIP, PO Box 435, 3219 Yonge Street, Toronto, Ontario M4N 3S1. OR MAC PRO MEMBERSHIP, 2316 Delaware Ave., PO Box 289, Buffalo, NY 14216-2687 Within a few weeks you will receive your card and you may begin using your membership benefits. Providing you with the products, information and service you need is our ongoing commitment. Membership duration is one year.
